You confirm that the information you submit is accurate to the best of your knowledge, but 5th Ave Music Studio does not guarantee that submitted information, scheduling outcomes, third-party systems, or response times will be complete, error-free, uninterrupted, or available at all times.

By continuing, you authorize the studio to contact you by email, phone, or text about your request, scheduling, billing, and follow-up service updates. Your online checkbox may be used as an electronic acknowledgment of these request terms.
